Our office will assist with VA forms, please call and make an appointment. If you decide to complete on your own, make sure to make copies of everything before sending into the VA for your records.

Send Enrollment 1010EZ or Renewal 1010EZR Applications to:

Compensation/Pension goes through a Service Organization Representative (form 21-22) you choose. The Representative you choose will forward all documents to the Regional Office. Service Organization Representatives are listed below (which are located in Des Moines, IA), you may only select one to represent you:

~ American Legion
~ Disabled American Veterans
~ Military Order of the Purple Heart
~ Paralyzed Veterans of America
~ Veterans of Foreign Wars
~ Vietnam Veterans of America

If the form or application requires a Certified DD214. Do not send your original DD214, as you will not get it back. If you have it recorded, contact the Recorder's Office for a Certified copy at no cost to the veteran. If you do not have it recorded, you should do so.
